Recognition as an Oregon State University (OSU) Icon of Pharmacy and induction into the Pharmacy Hall of Fame is reserved for OSU alumni or those who have demonstrated dedication to the College of Pharmacy and who have distinguished themselves through sustained and meritorious contributions to pharmacy throughout their careers.

The general criteria for the awards are indicated above. Detailed criteria consist of the following:
Nominations should be in writing (up to two pages), citing major accomplishments, important career milestones, honors received, personal characteristics and human interest aspects about the nominee that support the nomination. A résumé or vita is not required, but should be provided if available.
Nominations will be solicited by direct mailing to current Pharmacy Hall of Fame and College of Pharmacy Advisory Council members, as well as through solicitations in the BEAVERx newsletter and on the College of Pharmacy Web site. Nominations may be made by faculty, staff, alumni or other friends of the college. Written nominations are due Feb. 1 each year, signed by one or more nominators, and sent to Megan Hoffart, director of alumni and student relations, 203 Pharmacy Building, Oregon State University, Corvallis, OR 97331-3507. Nominations may also be sent by e-mail to megan.hoffart@oregonstate.edu.
Nominations will be reviewed by a subcommittee of the College Council (department chairs, associate/assistant deans, director of alumni and student relations, director of development), the chair of the College of Pharmacy Advisory Committee and one Hall of Fame member. The selection committee will make recommendations to the dean for final selection by March 15.
Up to six awards will be made each year. A compelling case for additional inductees will be considered.
All inductees will be invited to the annual Icons in Pharmacy recognition dinner to be held each spring and will receive a personalized plaque. Each will receive permanent recognition as a Hall of Fame member within the college through wall plaques and a brief citation displayed on our Web site. Awards will also be publicized through the BeaveRx Newsletter, the Oregon Stater and press releases.
