Leave of absence policy

The Doctor of Pharmacy (Pharm.D.) program is a four-year program. Students may take up to five years to complete the program with approval from the Academic & Professional Standards (APS) Committee. Any student wishing to use the fifth-year option (sometimes called “flexing”) must first petition the APS committee for approval. Please contact Angela Austin Haney, Director of Student Services/Head Adviser for specific instructions regarding the petition.

CPR/first aid policy

All students must have a current First Aid Certification and Adult CPR certification at the time of matriculation to the College of Pharmacy. Adult CPR certification that includes training on an Automatic External Defibrillator (AED) is recommended. Alternative certification may be substituted, but must be approved by the director of student services/head adviser. Online certification or recertification for CPR is NOT accepted.

CPR certification should be renewed annually and must be current while enrolled in the College of Pharmacy. Current OSU students can enroll in CPR/First Aid courses offered through Dixon Recreation Center. Information is available online.

Intern license information

All internship licensure paperwork must be obtained from the board of pharmacy for the state in which you plan to intern. Paperwork must be completed and submitted to Angela Austin Haney, director of student services/head adviser. Please allow at least one week for processing.

Graduation and licensure

Graduation audits are completed in January and February for all fourth-year (P4) students. Graduation audits are mailed out with graduation application forms in February. All graduating Pharm.D. students must fill out the graduation application regardless of their participation in any commencement ceremonies.

There are three opportunities for Pharm.D. students to participate in commencement exercises. The primary commencement ceremony is hosted by the College of Pharmacy on the Saturday before OSU’s official commencement ceremony. The COP event is a chance for all graduating Pharm.D. students to be hooded by the faculty, receive awards and reunite with their classmates. Pharm.D. students are also encouraged to participate in either the OHSU ceremony in early June or the OSU ceremony in mid-June. The official graduation date reported to the Board of Pharmacy is the OSU graduation date (held mid-June).

Once all graduating Pharm.D. students have completed their rotation requirements (block 8 is complete), the director of student services/head adviser will request that the OSU Registrar’s Office send a list of all graduating Pharm.D. students to the Oregon Board of Pharmacy. If any graduating Pharm.D. student needs notification of graduation sent to another state’s board of pharmacy, they should contact Angela Austin Haney, director of student services/head adviser.

Bachelor’s degree policy

Though students may enter the Pharm.D. program prior to obtaining their B.S. degree, it is required that their bachelor’s degree be completed before starting the P3 year. Upon completion of the bachelor’s degree, students should send official copies of transcripts showing the degree to the College of Pharmacy.

Pharm.D. students must keep their address updated in two places:

Insurance and student health services

First- and second-year (P1 and P2) Pharm.D. students are required to pay OSU student health fees. Third-year (P3) students are required to pay OHSU student health fees and mandatory health insurance fees. Fourth-year (P4) students can choose from several student health services depending on the location of their clerkship rotations. P4 students must choose OHSU student health fees if they have a rotation with OHSU. P4 student cannot choose to have the OHSU health insurance. Only P3 students are required to have health insurance although it is strongly recommended all Pharm.D. students have health insurance coverage.
