Timeline

The Early Admission Program (EAP) admission cycle for fall 2008 has concluded. Applications for fall 2009 are currently available, and will be due on March 2, 2009.

Application and instructions

• For complete EAP application materials, download the following:
  • 2009 EAP Application Form (and instructions)
  • 2009 EAP Confidential Reference Form
• The application materials are in PDF format. To print the application, you will need Acrobat Reader on your computer.

A complete EAP application must include:

• Early Admission Program Application form (above)
• Three short essays (instructions on the application form)
• Two sealed reference letters (instructions on the reference form above)
• Resume
• Official high school and college (if applicable) transcripts
• Application fee of $50

Submit all the components of your application in ONE PACKAGE. Applications must be RECEIVED at the College of Pharmacy by 5:00pm PST on March 2, 2009. Late applications will not be considered, even if post-marked by the deadline. Applications should be mailed to:
