In addition to the PharmCAS application, applicants should submit an OSU Supplemental Application. The OSU Supplemental Application is submitted online. A link to the OSU Supplemental Application will be emailed directly to candidates who have submitted an application on PharmCAS and who meet OSU's minimum science GPA standard (2.75 for Oregon residents and 3.0 for non-residents).

The link to the OSU Supplemental Application will be emailed after PharmCAS verifies your application. Applicants are responsible for ensuring that PharmCAS has all the information it needs to verify your application, and should check their application status on PharmCAS regularly.

Please add,,, and anything from the * domain to your contacts to ensure that you receive the email with the supplemental application, and follow all of PharmCAS's instructions regarding email addresses to add to your safe senders list.

The OSU Supplemental Application includes three parts: the OSU essays, the Prerequisite Worksheet, and the Supplemental Application Fee.

Essays: Applicants will be asked to write one or more short essays about their desire to attend OSU. Information about these essays will be included in the email with the link to the OSU Supplemental Application.

Prerequisite Worksheet: This worksheet, which is part of the online OSU Supplemental Application, asks applicants to list the coursework they have taken that meets the prerequisites for OSU's Pharm.D. program.  The chair of the Admissions Committee will email candidates with any questions or concerns related to prerequisite completion.  Please do not ask for a transcript evaluation at this point in the application process.  You courses will be evaluated after you submit the supplemental application.

Fee: The $50 application fee can be paid online with a credit or debit card. Information about the fee payment is included in the online OSU Supplemental Application. The fee is non-refundable.

Deadline: The deadline for completing the OSU Supplemental Application is 10 calendar days after the link to the Supplemental Application is emailed to you.

The Supplemental Application Fee is waived for applicants who have qualified for the PharmCAS fee waiver. After submitting your supplemental application, send an email to indicating that you have qualified for the fee waiver and are not submitting the Supplemental Appplication Fee. OSU will verify all fee waiver claims with PharmCAS.
