Information You Manually Provide. Oregon State University College of Pharmacy Office of Continuing Education collects Personal Information you manually provide. Generally, we use the Personal Information we collect through the Site to:

Use of Survey Information
On occasion, we may request your participation in a survey or questionnaire. You may receive an email with a survey link sent through Canvas LMS. We compile statistical information from the responses to our surveys. These statistics are not personally identifiable. These statistics help us understand the needs and preferences of the groups we serve.
Your Choices
We respect your right to make choices about the ways we collect, use and disclose your Personal Information. In most cases, we will ask you to indicate your choices at the time we collect your Personal Information. In some cases, if you decline our ability to collect, use, and disclose your personal information, you may not be able to register for and complete our online courses.

User and password information is encrypted before it is transmitted across the network.

Employers. We may disclose Personal Information collected through our Site with your employer. If your enrollment fees are sponsored by your employer, they may require us to provide your name, email, employee ID, store #, and registration & completion dates.
Accrediting Bodies. We may disclose Personal Information collected through our Site with an accrediting body such as ACPE, OHSU (CME), or OBMI.
We will not sell or share any personally identifiable information to any third parties not mentioned above.
