We want you to succeed as both a student and a professional pharmacist. These resources have been compiled to help you achieve success at school and afterward.
Current Pharm.D. students and alumni can apply to participate in the Pharm.D./MBA dual degree program. This program, offered in collaboration with the College of Business, allows students to work towards Pharm.D. and MBA degrees simultaneously. The amount of time and coursework needed to complete the MBA will vary depending on students' academic goals and prior business experience. Click here for curriculum information.

Current students interested in the Pharm.D./MBA program should meet with Lauren Corwin (Lauren.Corwin@oregonstate.edu) before submitting the interest form below. Alumni interested in the MBA program should contact Paige Clark, or send a letter to Oregon State University, College of Pharmacy, 203 Pharmacy Building, Corvallis, OR 97331.
Petition Form - This form is used to submit a petition regarding academic or professional policies of the college. Examples of reasons to create a petition include desiring to take a non-approved elective course or desiring to participate in the distance education program.
MBA Program - This form is used to express interest in the Pharm.D./MBA program. An academic advisor will follow up with you after you submit the form.
Elective Request Form (.DOC) - This form is used to request enrollment in a research-based PHAR 701, 705, or 797 class, ensuring that the student and his/her research mentor have a coherent plan of study.
Needle Stick/Bodily Fluid Incident Report Form - This form is used to report bodily fluid exposures, needle sticks, or similar injuries to College of Pharmacy faculty, staff, and students. Submit this form to the Director of Experiential Programs within 24 hours of the injury.
Academic Grievance - This form is used to submit an academic grievance to the College of Pharmacy Academic and Professional Standards Committee.
Incident Report Form - This form is used to submit an incident report the the College of Pharmacy Academic and Professional Standards Committee.
